Positive Downs Syndrome Ultrasound

A positive Down syndrome ultrasound indicates that certain markers or measurements suggest an increased likelihood that the baby may have Down syndrome. Dr. Hammond uses a systematic approach to identify markers for Down syndrome during the 12-week ultrasound:

Assessing the thickness of the fluid at the back of the baby’s neck. An increased NT measurement can indicate a higher risk of Down syndrome.

Checking for the presence of the nasal bone. The absence or underdevelopment of the nasal bone can be a soft marker for Down syndrome.

Evaluating blood flow from the placenta to the baby’s heart. Abnormal blood flow patterns can suggest heart defects associated with Down syndrome.

Observing the overall facial profile and head shape for abnormalities that might indicate chromosomal conditions.

Reviewing the heart’s structure, including the four-chamber view and cardiac outflow tracts, for any defects commonly associated with Down syndrome.

Looking for additional markers such as shortened femur or humerus bones, echogenic bowel, or pyelectasis (dilation of the kidneys).

By carefully evaluating these factors, Dr. Hammond can identify signs that suggest a higher likelihood of Down syndrome, allowing for timely follow-up and further testing to confirm the diagnosis.

Trisomy 21 Screening

During the 12-week scan, several key factors will be evaluated. They include:

The baby’s size is measured, and it should be between 48 and 79mm.

The thickness of the neck skin fold is measured. Babies with Down syndrome often have increased neck thickness due to a higher likelihood of heart defects and a connective tissue condition that retains fluid. Measurements above 3mm are concerning, although 85% of these cases will still have a normal chromosome profile.

The presence of the nasal bone is checked, as Down syndrome babies usually have very flat nose bridges, and only 3% will show a visible nasal bone. However, it is important to note that up to 15% of normal babies may not have a visible nasal bone at 12 weeks. The angle of the nasal bone in relation to the forehead is also observed.

This refers to the blood flow from the placenta through the umbilical vein, into the belly button, and to the liver. This structure shunts oxygen-rich blood directly to the baby’s heart and closes shortly after birth. A specific blood flow pattern is seen in babies with normal heart development, while abnormalities, such as reversed flow, may indicate Down syndrome or other heart defects.

The scan will also check for major and obvious defects, such as the absence of the top part of the skull, limb defects, cystic hygroma (a large neck swelling associated with a missing X chromosome), and other anomalies.

Why Should I Have Pregnancy Blood Tests?

Pregnancy blood tests are an essential part of prenatal care, offering valuable information about the health and development of your baby and yourself. Blood tests can detect genetic conditions like Down syndrome (Trisomy 21), check for infections, and assess the levels of vital substances such as iron, glucose, and thyroid hormones. These tests can be performed on the day of the ultrasound or starting from 10 weeks of pregnancy. Conducting the tests earlier can slightly improve their predictive value.

Dr. Hammond may conduct pregnancy blood tests to ensure comprehensive prenatal care. Your blood test results will indicate the risk level, expressed as a ratio (e.g., 1: xxx). A risk lower than 1:300 is considered “low risk,” while a risk higher than 1:300 is considered “high risk.” While we recommend invasive testing for risks higher than 1:300, the decision ultimately lies with the parents. They need to consider whether they want to undergo any testing, accept that a definitive diagnosis may not be possible, or proceed with invasive testing only if the risk exceeds 1:100. The most challenging decision might be whether to continue the pregnancy if Trisomy 21 is confirmed through amniocentesis.

At this stage, all major structures are developed, and you will be able to see the baby move, jump, and roll on the screen.

 

Hearing the fetal heartbeat for the first time is an awe-inspiring experience, knowing that this heart will likely beat for the next 80-90 years. We will also identify key structures, such as the stomach bubble, which confirms the baby can swallow fluid, indicating that the brain, nerves, and muscles are functioning correctly. The bladder’s visibility confirms the kidneys are processing fluids, although the mother handles all waste until after birth.

If the baby is in a favourable position, we may be able to guess the sex, but this is not guaranteed. Most importantly, this and every other ultrasound can help parents and siblings bond with the new baby, a benefit that cannot be measured or quantified.
